For example, for Leighton Hospital, please choose North West region followed by Mid Cheshire Hospitals NHS Foundation Trust.

Sometimes it can be difficult once discharged from hospital.

A major source of support when you are home from hospital is the British Red Cross Support At Home (SAH) Service. This is a free service, available for 2 weeks.

Please make sure you read page 2 to see all on offer!

In Nantwich and Rural we are very lucky to have some fantastic volunteer agencies where you can access support when back home :

For those living in and around Nantwich there is the volunteering group of Nantwich Buddies.

Click on the Buddies logo above for a FULL introduction, or visit their Facebook Page here.

In and around Audlem, we have ADCA (Audlem and District Community Action).

For an introduction to ADCA’s services please click here.

Both the above volunteer organisations can provide help with transportation needs to your local GP Practice.

If you need assistance with transport to a follow-up hospital appointment, then you can request the West Midlands Ambulance Patient Transport Service. For full details of this non-emergency ambulance service please click here.

In our Care Community we also have the innovative alternative to conventional bus services with a Go-Too. No timetables, the bus only moves when it’s ordered by you, like a taxi.
As of September 2024, for concessionaries, this service is free after 9:30 am!!

This is aimed at mobilising those who are able to get out and about but who don’t drive or are unable to access regular, timetabled public transport options. This might be a very important service for those recently discharged from hospital, living in rural areas, who would like to be independent.

This new travel service takes you where you want to go, when you want to go…
…to the shops, to meet friends, to health appointments and more, with other people going in your direction. Virtual bus stops will allow you to be picked up from close to home. It might also enable friends to visit you.

Please click on the image above to jump to ALL the information you need on how it works and a list of frequently asked questions. Essentially go-too is a transport-on-demand bus service operating within the boundary of the map shown below. You can travel anywhere within the dark green zone, or between the dark green zone and Nantwich in either direction. So, the only journey not permitted would be one solely contained within the light green zone of Nantwich (as regular bus services apply here).
