Finding and Arranging Social Care
November 15, 2021If it’s getting harder to manage you may be able to get the extra care and support you need through social care. Social care is a very broad topic so here we introduce some fundamental aspects of social care, starting with this very short video :
Where to start with working out what care you need?
Your first step should be to ask your local council’s social services department for a care needs assessment. They will identify where you might need extra support from social care and together you can agree on a care and support plan if appropriate.
In Cheshire East the First Point of Contact number for Social Services is 0300 123 5010. (See the box below for further details on Cheshire East).
In Shropshire the First Point of Contact number for Social Services is 0345 678 9044.
The Age UK website guides you through the following initial questions which can greatly inform you of the process :
- What to expect from a care needs assessment.
- The types of social care service – Homecare, Home adaptations, Care homes and Specialist housing.
- How does paying for care work?
- What is a care and support plan?
- How do personal budgets work?
- What support is available if I’m leaving hospital?
