Social prescribing works for a wide range of people, including those:

with one or more long-term condition,
who need support with their mental health,
who are lonely or isolated,
who have complex social needs which affect their wellbeing.

Please do encourage those who you think might benefit from a social prescription to contact their GP Practice reception to book an appointment with the Practice Link Worker.

Social prescription will work best when people can easily be referred to link workers from a wide range of local agencies including multi-disciplinary teams, hospital discharge teams, fire service, police, job centres, social care services, housing associations and voluntary, community and social enterprise (VCSE) organisations. This is something we are looking to introduce.
